Second, you can e-mail us at appointments@zeropointhealers.com . and we will be happy to schedule an appointment for you through e-mail correspondence.

Please remember to bring (or send) a signed copy of the Consent for Treatment form, the Disclosure form,  and a completed Confidential Client Intake Form to your first appointment.
